FIDI (Fundação Instituto de Pesquisa e Estudo de Diagnóstico por Imagem) is one of Latin America’s largest radiology service providers, delivering more than 400,000 diagnostic studies each month across 90 sites in Brazil. Supporting everything from full radiology operations to remote teleradiology, FIDI works with both public and private healthcare institutions, including mobile clinics that serve patients in
underserved regions.

In 2024, FIDI introduced Oxipit’s AI-powered chest X-ray support tool in emergency departments, where fast, consistent interpretation is critical and real-time radiologist input is not always available. The solution was deployed through CARPL— the world’s only US-FDA cleared and CE marked vendor-neutral AI platform that enables radiologists to seamlessly access, assess, and integrate AI applications into clinical practice.

Supporting ER physicians where time and clarity matter most

In Brazil’s busy emergency rooms, physicians are frequently required to interpret chest X-rays on their own. These clinicians — often early in their careers and hired through third-party agencies — must make quick decisions without the benefit of radiology reports. While experienced physicians can manage, the variability in training and high-pressure environment present a risk to diagnostic consistency.

“For emergency patients, speed is everything,” explains Dr. Osvaldo Landi Júnior, Medical Innovation and Data Manager at FIDI. “The physicians need results quickly, but they don’t always have the confidence or expertise to interpret chest X-rays accurately.”

This creates a three-option dilemma for ER staff:

  1. Wait for a radiologist report, which can take hours.
  2. Order additional imaging, like a CT scan, increasing costs and delays.
  3. Proceed with uncertainty, risking misdiagnosis or overtreatment.

None of these are ideal. FIDI needed a fourth option — one that could ensure consistent, fast, and interpretable diagnostic support at the point of care.

AI support for confident decisions

To address this, FIDI deployed Oxipit’s chest X-ray solution, which visually annotates potential findings directly on the image. Instead of producing standalone reports, Oxipit’s output overlays diagnostic indicators on the image itself — a format that proved ideal for clinicians needing immediate visual feedback.

Radiologists are not the primary users in this case. The client is the referring physician — ER doctors, general practitioners, surgeons.

Oxipit’s AI helps them see key findings instantly. It brings a new level of confidence and clarity to the decision-making process.

The seamless integration of CARPL’s AI platform with Philips’ Enterprise Imaging Solution (Vue PACS) allows Oxipit’s AI-generated annotations to appear directly within the radiologist’s or clinician’s existing workflow — without additional windows or clicks.

CARPL made the integration process remarkably smooth. With Oxipit already validated through CARPL’s proprietary testing and monitoring platform, FIDI could bypass a lengthy procurement and development cycle.

More than 4,500 studies were tested during validation, evaluating sensitivity, specificity, and other performance metrics. Once deployed, Oxipit’s AI began supporting chest X-ray interpretation 24/7 — ensuring faster insights and greater diagnostic consistency.

How Oxipit AI fits clinical needs — and delivers results

The rollout began gradually in April 2024, ramping up through June and going fully live by July. Oxipit’s AI has since become a key decision support tool in FIDI’s partner ER departments — particularly those without on-site radiologists.

With Oxipit, ER physicians now receive AI-annotated images in real time. This allows them to quickly assess critical findings, reduce reliance on additional imaging, and deliver care with greater certainty. It also gives hospitals peace of mind, knowing that a baseline level of quality and safety is maintained regardless of which third-party physician group is staffing the ER.

Oxipit allows us to guarantee a standard of quality. It doesn’t matter who’s on shift — everyone gets the same diagnostic support.

The visual format — markings directly on the image — has been especially well received. However, adoption challenges remain, particularly among newer physicians still building trust in AI tools. Some initial scepticism centered around concerns that AI might replace human expertise.

But FIDI’s approach is clear: this is about support, not substitution.

“AI is like a cooperation,” Dr. Landi Júnior explains. “The human brings in some skills, AI brings others. They complement each other.”

FIDI is planning additional training sessions to further support adoption, though the constantly changing ER staff — often managed by external companies — makes education logistics complex.

Scalable, practical AI adoption — one use case at a time

CARPL has played a vital role in turning FIDI’s AI vision into reality. Rather than investing in a large in-house innovation team, FIDI relies on CARPL for the discovery, validation, and deployment of imaging AI applications.

“We see CARPL as an extension of our innovation team,” says Dr. Landi Júnior. “They’ve helped us bring AI into production without heavy lift from our side. The pre-built integrations, the proactive support — it’s been a very positive collaboration.”

CARPL’s platform also helps FIDI ensure interoperability and seamless integration with existing PACS infrastructure, allowing the AI results to flow naturally into the diagnostic workflow.

There’s no turning back now,” says Dr. Landi Júnior. “AI will keep evolving. Even if you can’t adopt it today, don’t lose sight of it. Start small. Test it. Find where it fits. That’s how we approached it, and it’s working.

About FIDI

Founded in 1986 by medical professors from the Department of Diagnostic Imaging at the Escola Paulista de Medicina – now the Federal University of São Paulo (UNIFESP) – FIDI is a private, non-profit foundation that reinvests 100% of its resources into providing medical care for the Brazilian population. This is achieved through the development of imaging diagnostic solutions, medical education, research and
outreach activities, as well as social and philanthropic initiatives.

With more than 2,100 employees and a technical team of over 500 partner physicians, FIDI operates in over 90 healthcare units across the states of São Paulo, Goiás, Mato Grosso do Sul, Minas Gerais, and Rio de Janeiro. It is the largest diagnostic imaging company in Brazil. In 2024, FIDI performed 5 million exams — a 7% increase compared to 2023 — including MRI, CT scans, ultrasound, mammography, X-rays, bone densitometry, hemodynamics, and nuclear medicine.

With customized imaging diagnostics solutions, FIDI offers services in Teleradiology, Full Management, Consulting, Medical Education, and Artificial Intelligence. The Foundation is also committed to proposing innovative solutions for public healthcare, such as AI-based CT image analysis systems. It also participated in Bahia’s first public-private partnership (PPP) for diagnostic imaging. FIDI has received the Referências da Saúde award twice (2019 and 2020) in the Healthcare Quality category, and has earned three medals in international artificial intelligence competitions for diagnostic imaging at the annual conference of the
Radiological Society of North America (RSNA), the world’s largest radiology congress. At the end of 2020, FIDI’s Reporting Center received the ISO 9001:2015 Quality Management Certification, which was renewed in 2023. In 2021, FIDI was also recognized with the “Great Place to Work” (GPTW) certification. In 2025, FIDI was awarded the Health Leaders Award in the Laboratories category, a recognition by
Grupo Mídia honoring companies, industries, sectoral entities, and healthcare service providers.
Since 2014, FIDI has taken part in the “Mulheres de Peito” mobile clinic project, a partnership with the State of São Paulo, offering free mammograms. The initiative has reached over 300 municipalities, providing more than 360,000 mammograms, 19,000 ultrasounds, and 900 biopsies, with over 3,900 women referred for further care.

The Foundation expands its outreach each year with mobile initiatives that promote health and well-being. One such effort, in partnership with the Guarulhos City Hall (SP), is the “Women’s Health Truck” project, offering services such as bilateral mammography, breast ultrasound, and transvaginal ultrasound. These actions reinforce the Foundation’s commitment to improving healthcare access, ensuring quality care, and positively impacting the lives of thousands of people.
